New Agents:

Complete & Sign the W-9 Form
Fax or email the W-9 Form along with a copy of your E&O Declarations page and your Agent’s License to:

If you have a new business submission with completed applications, send it now, then just send us the W-9 and other required forms as soon as possible.

Capable of Fully Assessing Risk Profile

New Business Submission:

Completed Acord applications or pertinent details on the risks (Year built, construction, square footage, distance to hydrant, alarms, etc.) and insured information (full name, date of birth, drivers license number, claims and losses, etc.)
MVRs for all licensed drivers in household
Details on any losses (when, where, how much, what was done to prevent future losses)
PC 9/10 risk require the supplemental Fire Protection form.
Homes over 25 years old require the older home form or modernization form.

Required to bind:

Signed Acord applications for each line of business
Signed additional applications or questionnaires, as required by the carrier.
MVR and CLUE reports (sub agents need to supply these for underwriting)
Surplus lines risks require 100% premium payment prior to binding
